FORM RENTAL POLICY*

1. If customer does not have an open account, all form rentals must be paid in full BEFORE delivery or pick-up of said items.  Customers with open accounts will be charged a security deposit, as follows:

     Form Rentals up to $500 require a security deposit of $500.
     Form Rentals up to $1000 require a security deposit of $1000.
     Form Rentals over $1000 will be determined accordingly.

2. All forms must be cleaned prior to return and all nails removed.  NO EXCEPTIONS!
3. There will be a charge for all deliveries or pick-ups, including truck travel time from the time the truck leaves our yard until it returns to our yard.
4. Forms that are to be picked up must be stacked on a board and placed in an area accessible to our truck.
5. Forms that are scattered around, in the excavated hole, or against the foundation WILL NOT be picked up, plus you will be charged for the trip out.
6. Changes to original plans submitted (i.e. re-figuring) may be subject to additional charges.   Contractors submitting sketches are responsible for their accuracy.
7. At conclusion of rental, any adjustments to invoice/bill will be made.
